Submit only a completed BINGO board

Only fill out this form if you have a fully completed BINGO Board, as this form is meant to award only those that have successfully raised $136. For your fundraising money to be approved, you are required to let your donors know that they must put down your name. This must be done to ensure that each person that has raised $136, will get credit for their fully completed BINGO board, and be awarded for their contributions.*
